Why is form a question of its own?
Because the same fact can be recorded usefully or uselessly. Revenue grew strongly cannot be checked; quarterly revenue of $120M, up 40% year on year, announced today by the company can. Asking for the form separately makes the reader restate each fact as something with an actor, a number, a unit and a date — and label an opinion as one.
